What to Do When Your Kit Arrives
Your ALZ Blood Test kit includes everything needed for your blood draw and return shipment. Please follow the instructions below based on your situation.
Step 1: Do Not Discard Any Packaging
Your kit is packaged as a box within a box.
- Keep both boxes
- Do not throw away the outer shipping box
- The outer box will be used to return your sample
Step 2: Determine Where Your Blood Draw Will Take Place
Option A: Your Provider Is Performing the Blood Draw
If you ordered your test through a provider who will draw your blood:

Bring your entire kit to your appointment, including:
- Outer shipping box
- Inner kit box
- All contents
- Your kit includes:
- A required form
- All materials needed for the blood draw and return
- After your blood is drawn:
- The provider will package your sample
- The prepaid return label will be applied to the outer box
- The kit will be shipped back for processing
Option B: Your Provider Does NOT Perform Blood Draws
If your provider does not draw blood:
- Open your kit and locate the instruction sheet inside
- Find the QR code
- Scan the QR code to schedule your blood draw at one of our partnered phlebotomy locations

Once scheduled, bring your entire kit to your appointment, including:
- Outer shipping box
- Inner kit box
- All contents
- At your appointment:
- The phlebotomy provider will perform your blood draw
- They will package your sample
- They will apply the prepaid return label and ship it back
Step 3: After Your Blood Draw
- Your sample will be shipped to the lab for processing
- Typical turnaround time is within 3 business days after the lab receives your sample
Receiving Your Results
- You will receive an email with instructions to access your results
- This will include access to your ELIS account login
- Your results will be available securely through that portal
- Your provider will also receive your results
- If you have a follow-up appointment scheduled, they will review results with you then
- Otherwise, they may follow up as needed
Online Orders – Optional Follow-Up Support
If you ordered your test online (not through a provider):
- If you would like to review your results with a care team member:
- Your lab report will include a link to schedule a follow-up call
- You can choose an available date and time that works for you
- A team member will review your results and discuss next steps
Important Reminders
- Always bring the full kit to your appointment (both boxes and all contents)
- Do not discard any packaging
- The return label is included and must be used to send your sample back
- Your sample cannot be processed without proper packaging and documentation
